The IHSS Public Authority has put together the following information to explain changes to the provider enrollment process. Please take the time to read this information completely.
Enrollments and new provider orientations will be handled on a first-come, first-served basis.
All Enrollment and Orientations occur at 3700 Branch Center Road, Sacramento 95827
Note: OTHER IHSS OFFICES DO NOT HAVE THE MATERIALS TO PROCESS ENROLLMENTS
NEW PROVIDER ORIENTATIONS ARE TUES-TH/CHECK IN AT 8:30 A.M.
No-one will be admitted after 9:00 a.m.
Class is limited to the first 50 providers that arrive.
Effective 11/1/09, the Sacramento County IHSS Public Authority took responsibility for the new provider enrollment activities mandated by changes in state law. The new responsiblities are as follows:
New Providers (You are a new provider if you were not paid by IHSS prior to 11/1/09.)
- Complete a provider enrollment form (SOC 426).
- Attend a mandatory 1 1/2 hour orientation.
- Complete the provider enrollment agreement (SOC 846) stating that you understand and agree to the IHSS program rules and requirements.
- Present, in person, an original unexpired government issued photo ID and Social Security card with any required Right to Work documentation.
- Ensure that your consumer completes and signs recipient designation form (SOC 426A) and caregiver add/delete form (SAS 1207).
- Pass a Department of Justice LiveScan background check
- Note - all of these steps are to be completed through the Sacramento County IHSS Public Authority.
Current Providers (You are a current provider if you were paid by IHSS prior to October 31, 2009).
Complete a provider enrollment form (SOC 426).
Review the orientation materials provided by the state and complete the provider enrollment agreement (SOC 846) stating that you understand and agree to the IHSS program rules and requirements.
- Present, in person, an original unexpired government issued photo ID and Social Security card with any required Right to Work documentation. (Please note that there are shorter waiting times for afternoon walk-in appointments).
- Pass a Department of Justice LiveScan background check
- Important Note - The State of California has indicated that providers whose enrollments are not processed and complete by December 31, 2010 will not be paid. The Department of Justice has issued an advisory that it may take as long as one month to process your background check. DO NOT DELAY YOUR ENROLLMENT ACTIVITIES!
